Confidential services are provided at no cost by UUH Outreach’s small, multi-disciplinary (social work/nursing) team. Program staff partner with a diverse population of adults aged 60+ to support them in their efforts to age safely, with dignity, in their own homes and communities. PURPOSE As part of the Outreach team, the Social Services Supervisor is responsible for completing in-home psychosocial assessments of older adult residents of the Northwest Philadelphia Outreach catchment area, developing strategies to build on client strengths and overcome challenges, linking older adults and their caregivers to service agencies or programs, leading academic and community partnerships, supervising staff and interns, and furthering the development of the Outreach Program. PURPOSE As part of the Outreach team, the Community Nurse is responsible for completing in-home assessments of older adult residents of the Northwest Philadelphia Outreach catchment area, devising and implementing patient education efforts, linking older adults and their caregivers to service agencies or programs, assisting in community health promotion events, performing regular blood pressure screenings in the community, measuring outcomes of interventions, and furthering the development of the Outreach Program. The nurse will work with local congregations, aging-service agencies, healthcare providers, and other community organizations to identify older adults with non-skilled needs who may benefit from periodic assessment, monitoring, and/or information and referral.
