ORGANIZATION
Unitarian Universalist House Outreach Program (UUH Outreach) is a non-denominational, community-based program for older
adults living independently in Northwest Philadelphia. Confidential services are provided at no cost by UUH Outreach’s small,
multi-disciplinary (nurse/social work) team. Program staff partner with a diverse population of adults aged 60+ to support them
in their efforts to age safely, with dignity, in their own homes and communities. UUH Outreach values the many strengths of the
clients they serve, and recognizes their right to self-determination when accessing assistance.
PURPOSE
As part of the Outreach team, the Community Nurse is responsible for completing in-home assessments of older adult residents
of the Northwest Philadelphia Outreach catchment area, devising and implementing patient education efforts, linking older
adults and their caregivers to service agencies or programs, assisting in community health promotion events, performing regular
blood pressure screenings in the community, measuring outcomes of interventions, and furthering the development of the
Outreach Program. The nurse will work with local congregations, aging-service agencies, healthcare providers, and other
community organizations to identify older adults with non-skilled needs who may benefit from periodic assessment, monitoring,
and/or information and referral.
The ideal candidate is flexible, a strong communicator and collaborator, and a good listener. This person is also able to develop
trusting relationships with colleagues and clients, and views healthcare through a broad lens, with an interest in
understanding the impact of environment on health, and in helping clients navigate a healthcare environment that often
presents obstacles to accessing care.
*Please note that this position may be hybrid at this time, with both remote and in-person opportunities, guided by CDC
recommendations and other information to ensure safe operations and limited risk of COVID-19 transmission for our team
members and clients. The person in this position is expected to comply with all organizational policies and procedures,
including existing and evolving guidelines for office and community/home safety during the COVID-19 pandemic.
This role includes:
Conducting psychosocial assessments and general visits in client’s homes, the Outreach office, or via phone/Zoom, depending
on COVID-19 conditions, as determined by Executive Director
Serving as the first point of contact for referrals to the agency
Scheduling and coordinating initial assessments for the nurse and social work team
Developing and providing individual and group health/wellness education for clients and caregivers
Performing blood pressure screenings in homes and group settings
Learning about the full scope of client resources, programs, and services
Advocating for clients and helping them access the services for which they are eligible
Collaborating with healthcare providers and other agencies to maximize client services and support
Serving as agency coordinator for pet therapy partnership when applicable
Delivering food, supplies, and medical equipment as needed
Creating and updating the case record in electronic database on a timely basis
Contributing to development of Outreach programs
Participating in agency initiatives, meetings, and trainings
Preparing statistics as directed; reporting on activity outcomes to staff and Board
Working with student interns from various disciplines

REQUIREMENTS
Graduate of an approved school of nursing
Current professional registered nurse license in the Commonwealth of PA
Proof of COVID-19 vaccination
Knowledge of community health nursing and health promotion and/or interest/willingness to learn
Minimum five years’ experience working with older adults
Interest in working with a diverse population of older adults; experience preferred
Proficiency in Microsoft Office suite of programs; willingness to learn Apricot database system
Capacity to participate in Zoom and other types or remote, screen-based meetings
Valid driver’s license; ability and willingness to drive throughout Northwest Philadelphia; daily access to a reliable vehicle;
safe driving record
Ability to travel independently to perform home visits
Ability to hear and see or to use prosthetics that enable these senses to function adequately to assure that position
requirements are fully met
Able to traverse buildings and climb and descend stairs.

Dedicated work area that is separated from other living areas and allows for confidential communication
Ability to keep confidential agency and client information secure
Access to a high-speed internet connection; ability to use a work-issued laptop or other device
